I declare that prior to applying for the course I have read and accepted the Regulations on Internet Training Courses available here, including the provisions on the processing of personal data. Applying for the participation in the training course in a manner provided for in the Regulations is tantamount to concluding the Agreement with the Organizer and entails the obligation to pay the training course fee.

I hereby agree that any invoice in electronic format will be sent via email from the email address of the specific Deloitte company that organizes the training (which email address will be indicated in the training registration confirmation) to the email address indicated by the Client in this form.

I understand that pursuant to the Act on Statutory Auditors, Audit Firms and Public Oversight of 11 May 2017, an Audit Committee in a PIE is obliged to determine policies of taking training offered by a firm that carries out statutory audit of its financial statements and by its network companies. I represent hereby that prior to enrolling to the training I have read the independence principles and policies of the PIE I am employed at or otherwise involved in and of its subsidiaries and that I can participate in the training organised by Deloitte network companies.
